Supplemental Health Coverage
Added financial protection you can pair with your medical plan.
You’re eligible to enroll in supplemental health coverage if you’re a full-time employee, or a part-time employee working at least 30 hours per week.
These supplemental medical coverage plans through Unum pay a cash benefit directly to you to help pay for expenses not covered by your medical plan:
- Accident Insurance
- Critical Illness Insurance
You can enroll in one or both plans.
Accident Insurance
If you have a covered accident or injury, from common injuries to more serious events, Accident Insurance pays a set amount based on the injury and treatment, on top of any other insurance, and you can use it for out-of-pocket costs like copays and deductibles.
You can cover your spouse and children up to age 26, regardless of marital or student status.

Critical Illness Insurance
Critical illness insurance pays a lump sum if you are diagnosed with a covered illness. You can use the money however you choose, including out-of-pocket costs like copays and deductibles.
Covered illnesses include but aren’t limited to:
- Cancer
- Heart attack and stroke
- Organ failure
- Coronary artery disease
- ALS, MS, and Parkinson’s disease
- Dementia

Be Well Benefit
Each covered family member can earn $50, $75, or $100 a year, tied to your coverage level, for completing an annual screening such as a physical, mammogram, or flu shot. File your claim at unum.com, by mail, or by phone at 1-800-635-5597.
Cost for coverage
You pay monthly premiums for supplemental health coverage through payroll deductions. The amount you pay in premiums depends on:
- Accident Insurance: which plans you choose and who you cover.
- Critical Illness Insurance: your age and the amount you elect.
How to enroll
You can enroll in supplemental health coverage in ADP [NEED LINK] within 30 days of your hire date or qualifying life event.
Coverage starts the first of the month after your hire date.
There is no evidence of insurability for Critical Illness Insurance if you enroll at initial eligibility; enrolling later requires it.
